PayByPlateMA Foms – PBP Account Update Form
The account update form updates the payment method, new address, telephone number, email address, contact person, PIN, and vehicle information.
If you want to change any of these, you must print this form, fill it out, and send it to the EZDriveMA customer service centre.

The hearing request form link is here. Anyone requesting a formal hearing to dispute an invoice can easily print it.
After taking a printout, fill in your information carefully and mail it to the EZDriveMA Customer Service Center or fax 1-508-786-5222.
Lease/ Rental Reassignment Form
Rental/Lease companies search this URL (Check here) to any browser and print the form, fill the form, and mail or fax it to “Lease / Rental Reassignment P.O. Box 8007 Auburn, MA 01501-8007 1-877-627-7745 1-508-786-5222 (Fax)”. To successfully transfer your responsibility of toll to another party.
The required details are the Invoice number, Transaction information (including posting date, transaction date/time, license plate state, license plate, and toll amount), name, address, city, state zip code, and lease/ rental start and end date/time.
